The 10th chapter of Uttaratantra of Sushruta Samhita is named as Pittabhisyanda Pratisedha Adhyaya. This chapter deals with the treatment of Abhisyanda of Pitta Origin.
अथातः पित्ताभिष्यन्दप्रतिषेधं व्याख्यास्यामः ||१||
यथोवाच भगवान् धन्वन्तरिः ||२||
We will now expound pittabhisyanda pratisedha – treatment of Abhisyanda conjunctivitis) caused by Pitta; as revealed by the venerable Dhanvantari.
Abhisyanda and Adhimantha
पित्तस्यन्दे पैत्तिके चाधिमन्थे रक्तास्रावः स्रंसनं चापि कार्यम् |
अक्ष्णोः सेकालेपनस्याञ्जनानि पैत्ते च स्याद्यद्विसर्पे विधानम् ||३||
Below mentioned are the treatment methods and principles recommended for Abhisyanda and Adhimantha caused by vitiated pitta –

– Raktasrava – bloodletting
– Sramsana – purgation
– Aksi seka / pariseka – irrigation / pouring of medicated liquids to the affected eyes
– Lepana – application of medicinal pastes
– Anjana – collyriums, eye salves
– Paitte cha visarpa vidhanam – all treatments and comforts recommended for treatment of pittaja visarpa as explained in Chikitsa sthana section, chapter 17
Gundradi ghrta
गुन्द्रां शालिं शैवलं शैलभेदं दार्वीमेलामुत्पलं रोध्रमभ्रम् |
पद्मात्पत्रं शर्करा दर्भमिक्षुं तालं रोध्रं वेतसं पद्मकं च ||४||
द्राक्षां क्षौद्रं चन्दनं यष्टिकाह्वं योषित्क्षीरं रात्र्यनन्ते च पिष्ट्वा |
सर्पिःसिद्धं तर्पणे सेकनस्ये शस्तं क्षीरं सिद्धमेतेषु चाजम् ||५||
योज्यो वर्गो व्यस्त एषोऽन्यथा वा सम्यङ्नस्येऽष्टार्धसङ्ख्येऽपि नित्यम् |६|
Gundradi Ghrta is a praised medication for Pittaja Abisyanda. The ingredients, method of preparation and benefits are as below mentioned –
Ingredients – Paste and decoction is prepared with Gundra, Sai, Saivala, Sailabheda (Pasanabheda), Darvi, Ela, Utpala, Rodhra, Abhra (Musta), Padmaptra, Sarkara, Darbha, Iksu, Tala, Rodhra, Vestasa, Padmaka, Draksa, Ksaudra (honey), Candana, Yastika, Yositkshira (woman’s milk), Ratri and the two Ananta.
Ghee prepared from goat’s milk is then added to the above-mentioned decoction and cooked.
The resultant medicated ghee is called as Gundradi Ghrta.
Benefits – This ghee is used for –
– Tarpana – pooling around the eyes
– Pariseka – Eye bath or irrigation
– Nasya – nasal drops
Other preparation – Goat’s milk processed with the above said Gundradi Kalka – paste of Gundra etc herbs as explained above can also be used for tarpana, pariseka and nasya.
Also – Ghee shall be prepared using the above mentioned (Gundra etc herbs) herbs, all or as many as available, shall be used for administering four kinds of Nasya i.e. Pratimarsa, Marsa, Avapida and Sirovirecana Nasya.
Pittahara Kriya
क्रियाः सर्वा पित्तहर्यः प्रशस्तास्त्र्यहाच्चोर्ध्वं क्षीरसर्पिश्च नस्यम् ||६||
All pittahara kriyas – treatments, medicines, foods, activities and comforts which are helpful in mitigating pitta shall be used in treating pittaja abhisyanda. Milk and ghee may be used after three days. They can be as food inclusions and also to administer as nasal drops.
Recipes for Anjana and Rasakriya
पालाशं स्याच्छोणितं चाञ्जनार्थे शल्लक्या वा शर्कराक्षौद्रयुक्तम् |७|
रसक्रियां शर्कराक्षौद्रयुक्तां पालिन्द्यां वा मधुके वाऽपि कुर्यात् ||७||
मुस्ता फेनः सागरस्योत्पलं च कृमिघ्नैलाधात्रिबीजाद्रसश्च |
Below mentioned recipes of Anjana and Rasakriya are beneficial for application in pittaja abhisyanda –
For Anjana (collyrium) – Palasa, Sonita (kumkuma) or Sallaki mixed with sugar and honey.
For Rasakriya (thickened decoction) – Palindi or Madhuka added with sugar and honey.
For the Rasakriya – Musta, Samudraphena, Utpala, Krimighna, Ela, Dhatri and Bijaka.
Other four Anjana recipes for usage in Pittaja Abhisyanda
तालीशैलागैरिकोशीरशङ्खैरेवं युञ्ज्यादञ्जनं स्तन्यपिष्टैः ||८||
चूर्णं कुर्यादञ्जनार्थे रसो वा स्तन्योपेतो धातकीस्यन्दनाभ्याम् |
योषित्स्तन्यं शातकुम्भं विघृष्टं क्षौद्रोपेतं कैंशुकं चापि पुष्पम् ||९||
रोध्रं द्राक्षां शर्करामुत्पलं च नार्याः क्षीरे यष्टिकाह्वं वचां च |
पिष्ट्वा क्षीरे वर्णकस्य त्वचं च तोयोन्मिश्रे चन्दनोदुम्बरे च ||१०||
- Anjana – prepared by macerating Talisa, Ela, Gairika, Usira and Sankha in breastmilk.
- Anjana – prepared by macerating smooth powder or juice / decoction of Dhataki and Syandana with breastmilk.
- Anjana – prepared by macerating Satakumbha (gold) with breastmilk and added with honey.
- Anjana – prepared by macerating Kimsuka flowers with breastmilk and added with honey.
- Anjana – prepared by macerating Rodhra, Draksa, Sarkara, Utpala, Yasti, and Vaca with breastmilk.
- Anjana – prepared by macerating bark of Varnaka, Candana and Udumbara with cow’s milk and added with water.
Other useful recipes for Anjana and Ascotana
कार्यः फेनः सागरस्याञ्जनार्थे नारीस्तन्ये माक्षिके चापि घृष्टः |
योषित्स्तन्ये स्थापितं यष्टिकाह्वं रोध्रं द्राक्षां शर्करामुत्पलं च ||११||
क्षौमाबद्धं पथ्यमाश्च्योतने वा सर्पिर्घृष्टं यष्टिकाह्वं सरोध्रम् |
तोयोन्मिश्राः काश्मरीधात्रिपथ्यास्तद्वच्चाहुः कट्फलं चाम्बुनैव ||१२||
Anjana, prepared by macerating Sagaraphena with breastmilk and added with honey.
Yasti, Rodra, Draksa, Sarkara and Utpala are kept immersed in breastmilk for some time. Later, they are ground and made into paste. This paste is used as Anjana in pittaja abhisyanda.
Powder of Yasti, Rodra, Draksa, Sarkara and Utpala tied in a linen cloth (silk) is dipped in breastmilk repeatedly. This milk is used as aschyotana – eye drops.
Powder of Yasti and Rodra macerated in ghee should be used as Anjana or Aschyotana.
Fine powder of Kasmari, Dhatri and Pathya should be tied in a sterile cloth and a bolus is prepared. This bolus is dipped in water. This water is used as Ascyotana. Alternatively, the powders of the mentioned herbs should be macerated in water and used as ascyotana.
Fine powder of Katpala is tied in a sterile cloth and bolus prepared. This bolus is dipped in water and that water is used as eye drops. Alternatively, Katpala is macerated with water and the juice expressed is dropped into the eyes as ascyotana.
Amladhyusita and Suktika Cikitsa
एषोऽम्लाख्येऽनुक्रमश्चापि शुक्तौ कार्यः सर्वः स्यात्सिरामोक्षवर्ज्यः ||१३||
सर्पिः पेयं त्रैफलं तैल्वकं वा पेयं वा स्यात् केवलं यत् पुराणम् |
दोषेऽधस्ताच्छुक्तिकायामपास्ते शीतैर्द्रव्यैरञ्जनं कार्यमाशु ||१४||
All the above-mentioned treatments should also be adopted in the treatment of Amladhyusita and Suktika eye disorders. Bloodletting by siramoksha i.e. venepuncture should be avoided in these cases.
Below mentioned ghee can also be given in these conditions –
– Triphala ghrta,
– Tilvaka Ghrta or
– Kevala Purana Ghrta – Plain old ghee
In Suktika, –
– Virecana – purgagtion should be administered to eliminate the causative morbid dosha
– Anjana prepared using herbs of cold potency should be quickly administered into the eyes
Anjana for Suktika
वैदूर्यं यत् स्फाटिकं वैद्रुमं च मौक्तं शाङ्खं राजतं शातकुम्भम् |
चूर्णं सूक्ष्मं शर्कराक्षौद्रयुक्तं शुक्तिं हन्यादञ्जनं चैतदाशु ||१५||
Below mentioned ingredients should be made into nice powder –
– Vaidurya (cats eye / beryl),
– Sphatika (alum),
– Vidruma (Coral),
– Mukta (pearl),
– Sankha (conch),
– Rajata (silver) or
– Satakumbha (gold)
This powder should be added with sugar and honey and mixed thoroughly. This recipe should be used as Anjana. It cures Suktika eye disease quickly.
Dhumadarsi Cikitsa
युञ्ज्यात् सर्पिर्धूमदर्शी नरस्तु शेषं कुर्याद्रक्तपित्ते विधानम् |
यच्चैवान्यत् पित्तहृच्चापि सर्वं यद्वीसर्पे पैत्तिके वै विधानम् ||१६||
Below mentioned are the treatment principles of Dhumadarsi eye disorder –
– Sarpi – adequate amount of ghee needs to be used in the form of food (along with food) and also as ascyotana – eye drops
– Raktapitta vidhanam – all treatments, medicines and comforts recommended for Raktapitta (bleeding disorders) can also be used in treatment of Dhumadarsi
– Pittahrt Kriya – all pitta mitigating measures or therapies
– Paittika Visarpa Vidhanam – treatments, medicines, recipes and comforts indicated in Pittaja Visarpa should also be applied in treating dhumadarsi
इति सुश्रुतसंहितायामुत्तरतन्त्रान्तर्गते शालाक्यतन्त्रे पित्ताभिष्यन्दप्रतिषेधो नाम दशमोऽध्यायः ||१०||
Thus ends the tenth chapter by name Pittaabhisyanda Pratisedha in Uttara Sthana of Susruta of Samhita.
