Antra Vrddhi means herniation of the intestine into the scrotum. Other than Antra Vrddhi, the remaining six kinds of Vrddhi (explained in chapter 12 of Nidana Sthana section of the same treatise) are curable.

Patients of Vrddhi should avoid the below mentioned things –

riding on animals like horses etc.

        physical exercises,

        copulation,

        suppression of urges,

        sitting for long periods,

walking for long periods of time,

        fasting,

consumption of heavy (indigestible or difficult to digest) foods

Snehana – Mixture of three fats i.e. oil, ghee and muscle fat should be given for drinking to the patient having scrotal swelling produced by aggravated vata.

Swedana – He should also be given sudation therapy in the usual way.

Virechana – After Snehana – oleation and Swedana – sudation, the patient should be administered purgatives. One of the below mentioned purgative recipes should be given –

        oil of kosamra,

        oil of tilvaka,

        oil of errands phala or

        eranda taila mixed with milk

These recipes shall be given daily, for a time period of one month.

Vasti – Next, decoction enema should be administered. The enema liquid should be prepared with decoction and paste of vata mitigating herbs.

The patient is advised to take food along with meat soup.

After that he should be administered with oil enema. For this, Yastimadhu Taila should be used.

Other measures – Below mentioned measures should also be administered –

        anointing with oils,

        warm poultices

application of pastes prepared with vata mitigating herbs

The swelling which is ripening should be made ripe. After ripening it should be cut open taking care to avoid the suture (scrotal raphae). Following this, treatments recommended for wounds described previously should be followed.

For treating the ama (unripe) stage of scrotal swelling produced by pitta, the same treatments prescribed for pittaja granthi should be adopted.

On the other hand, if the swelling has ripened, it should be punctured (or incised).

The wound should be purified using a mixture of honey and ghee.

After this, the physician should bring about healing of the wounds by using oils and pastes prepared from healing herbs.

The physician should administer the below mentioned therapies in scrotal swelling caused by vitiated blood –

should remove blood (conduct bloodletting) using leeches or

administer purgation recipe added with honey and sugar

Treatments indicated for pittaja granthi should be administered in both ripening and ripe stages of the disease.

For treating Vriddhi caused by vitiated kapha, paste of herbs having hot potency macerated in gomutra – urine of cow should be applied.

For oral consumption, decoction of Pitadaru i.e. Daruharidra (Berberis aristata) should be used along with gomutra – urine of cows.

Except Vimlapana (kneading), the other treatments prescribed for Kaphaja Granthi should be used for treating Kaphaja Vrddhi.

The physician should incise the ripe swelling.

The wound should be made to heal using oil boiled and processed with the below mentioned herbs –

        Sumana,

        Aruskara,

        Ankotha and

        Saptaparna

Medoja Vrddhi i.e. vrddhi caused by morbid fat should be given fomentation.

It should be applied with paste of herbs belonging to Surasadi Gana or herbs belonging to Sirovirecana Gana macerated in Gomutra – cow urine. The application should be warm.

The swelling should be wrapped with cloth bands after administering fomentation. Later, it should be incised using Vrddhipatra – a sharp instrument, while protecting the testes and raphae. The fat therein is removed by scraping.

After this, the powder of Kasisa and Saindhava is applied. Bandaging is done as suitable.

The medicated oil prepared with manashila, ala, lavana and aruskara should be used for promoting healing.

For treatment of Vrddhi caused by urine i.e. hydrocele, fomentation should be first given. It should now be wrapped with cloth band following which puncture is made beside the raphae, below the swelling.

A tube with opening at both ends should be taken. It is inserted and the urine is drained out. Afterwards, the tube is removed. Sthagika type of bandage is applied.

It is made to heal as usual after cleaning the vrddhi as described.

Vrddhi caused by añtra i.e. intestinal hernia should generally be rejected (should not be treated).

Treatments to mitigate the aggravated vata are beneficial and should be done when the intestine has not descended into the scrotum.

When the vrddhi is localized in the groin i.e. inguinal hernia, it should be cauterized (branded by fire) with curved iron rod which is semilunar in shape. This would block its passage into the scrotum. The vrddhi should be rejected if it has descended into the scrotum.

A cut or incision should be made in the skin of the middle of the thumb (big toe). Cauterization should be done in the opposite part i.e. big toe opposite to the side of swelling.

The same methods of treatment should be adopted in vriddhi caused by vata-kapha and the area is cauterized with great effort. This should be done because there are great chances of cutting off the ligaments herein.

The vein present between the temples and ear should be punctured in the opposite side. While doing the puncture, the sevani i.e. suture on the skull should be avoided (care should be taken that it is not injured).

In the treatment of curable forms of Upadamsa (venereal diseases), the physician should at first administer oleation and sudation therapies.

After this, the vein at the centre of the penis should be punctured or bloodletting should be done using leeches.

Elimination therapies to expel the greatly aggravated doshas should be administered in both directions i.e. both vamana – emesis and virecana – purgation should be given.

Pain and swelling would subside by doing so.

The greatly increased doshas should be expelled by administration of decoction enema if the person is debilitated or did not have proper purgations.
